ASEBA

The Achenbach System of Empirically Based Assessment (ASEBA) is a collection of questionnaires used by Fountain Gate counselors to assess child and adolescent behavior. Evaluations are completed by parents, teachers, and, in the case of adolescents, by the clients themselves. The ASEBA system has been under development for over 40 years and is a highly respected and widely used instrument in mental health and school settings; it has been translated into over 80 languages. We employ the following ASEBA questionnaires:
Child Behavior Check List (CBCL): Completed by parents, for children aged 6-18.
Teacher's Report Form (TRF): Completed by teachers of childen aged 6-18.
Youth Self-Report (YSR): Completed by children aged 11-18.
Your counselor will provide you and your child's teacher with instructions and login information for completing the relevant ASEBA forms online. After receiving the instructions you may proceed to the online questionnaire and complete it. The results will be sent to your counselor who will discuss them with you in future sessions.
